In case of a medical, fire, or police emergency, you must be prepared to give the following information:

1. Type of emergency (medical or fire*)

2. Location of emergency (building name; location of building–streets and/or landmarks; floor and room number)

3. Telephone number from where call is being made

4. Caller’s name

5. What happened

6. How many victims

7. Condition of victim(s)

8. Help (First Aid) that is being given

–Let the people who answer the telephone hang up first!

–Have someone meet the emergency crew outside the building if possible

*In the event of a fire, pull the fire alarm, if possible; immediately leave the building and make the emergency call from another telephone in a safe location.

How to report an injury or accident, as a follow-up, not during the actual emergency:

If any employee is injured on university-owned or occupied space, a Cornell University Accident Report should be completed within 24 hours of the time of the accident. This form, with instructions for its completion, is available from your department administrator.
